To implement regional e-referral solutions that include precise information about the patient’s condition and the nature of the request to improve the handling of referrals and time to see the patient.
Scope
Public hospital inpatient and outpatient departments and clinics.
How SNOMED CT will be used
To create a coded clinical impression and reason for referral on every e-referral. The clinical impression will be entered by the referring clinician and may be modified at triage and at subsequent encounters.
Why SNOMED CT will be used
SNOMED CT enables precise information to be captured and acted on. Clinical pathways and clinical decision support can be added to SNOMED CT enabled e-referral apps.
